Documents and forms marked with an * are sent home in hard copy form with all students at the beginning of each year.  The remaining forms are no longer being provided in hard copy by the District.  We're working to preserve resources, reduce costs, and eliminate unnecessary paper documents.  Individuals should print and submit these other forms only if needed.
